HIPAA Policy
About health information privacy
The Health Insurance Portability and Accountability Act, commonly called HIPAA, establishes federal protections for health information held by covered healthcare providers, health plans, and certain other organizations. This page is a general explanation of patient privacy rights. It is not the practice’s formal Notice of Privacy Practices and does not replace that notice.
Ask for the current privacy notice
Ask Renew Wellness & Regenerative Medicine for its current Notice of Privacy Practices. That notice explains the practice’s uses and disclosures of protected health information, its privacy responsibilities, your rights, and how to make requests or complaints. You may request a copy at any time. An acknowledgement that you received a notice is different from an authorization to share your information for a particular purpose.
How HIPAA generally applies
Protected health information can include identifiable medical records, billing details, and information discussed during care. Where HIPAA applies, it permits certain uses and disclosures for treatment, payment, and healthcare operations, as well as other situations allowed or required by law. Other uses may require a written authorization. The practice’s current notice provides the applicable details and exceptions.
Your health information rights
HIPAA generally gives patients the right to:
- Request access to, or a copy of, information in their health records.
- Request an amendment if information is incorrect or incomplete.
- Ask for confidential communications and certain restrictions on uses or disclosures.
- Request an accounting of certain disclosures.
- Receive a privacy notice and raise a privacy concern.
These rights have specific procedures and legal limits. For example, requesting an amendment or restriction does not mean every request must be granted. Contact the practice for the appropriate process rather than sending medical records through a general website inquiry.
Website and email communications
Please do not submit Protected Health Information through this website. Call the practice to ask how to communicate about medical records, treatment details, or other sensitive information. General email is not the same as a dedicated patient communication system. The website Privacy Policy addresses routine inquiries and should be read alongside the practice’s formal health privacy notice.
Privacy questions or complaints
Contact the practice to request its notice or ask whom to speak with about a privacy concern. You may also file a complaint with the HHS Office for Civil Rights about a possible HIPAA violation. Complaints generally must be filed within 180 days of when you knew the event occurred, although HHS may extend this period for good cause. HIPAA prohibits retaliation for filing a complaint.
